Undefined error post upgrade to Remedy 7.6 IM

Hello,

 

We are currently using Remedy 7.6 intergrated with AP version 4 build 617. We are experiencing an issue when an incident is assigned to someone the incident is passed through without any issues. If the incident is updated, set to planning or entry added into the work log) another AP FYI alert is generated and in some cases this alert causes UNIDENTIFIED to appear n the email field. Why is happening when the original event passed through to AP ok?

 

1) Any thoughts on why the UNIDENTIFIED fields are appearing?

2) How do I disable the FYI alerts on updates as a test?

3) Should FYI email alerts have response choices as below?

 

Below is an example of the output of the report in the event activity...

Any of the events without any detail produce an email like below....

I have also attached AP logs and remedy script Log.

 

Any help would be appreciated.
